| General Credit Approval Information |
| This document will detail the steps necessary for a specific course pre-approval, a program approval, SM credit approval and course verification. |

| Course Pre-Approval Form |
| Please use this form to obtain pre-approval for a single course. Please remember to submit it on or before the first day of class. |

| Master's Degree Pre-Approval |
| Please use this form prior to starting your master's program. By attaching a list of all your courses to this form you avoid having to complete a pre-approval form for each course. |

| SM Credit Approval Form |
| Please use this form when requesting SM credits. You will also need to submit a course pre-approval form at the same time. |
|
| Verification Form |
| This is the form necessary to submit, along with original transcripts, when your course is completed. It must be turned no later than one year after the date of the last class. |

| SM Credit Verification Form |
| Please use this form when you have completed work on SM credits. You will also need to submit the course verification form at the same time. |
