1.  Mail It 

Mail the completed registration form with payment to:

     South Milwaukee Recreation Department

     901-15th Ave

     South Milwaukee, WI 53172

2.  Walk In 

South Milwaukee Recreation Department is located in the Senior High School at 901-15th Ave; South Milwaukee, WI 53172.  Use door #24 located on the east side of the Senior High School.  Regular office hours are from 8:00-Noon & 1:00-4:30 pm.

3.  Fax It

You may fax your registration form using a credit card.  Our fax number is:  414-766-5085.

The South Milwaukee Recreation Department is pleased to offer On-Line Registration for a varity of programs.  You may register from the convenience of your own home or office 24/7 at www.smrecdept.org.

5.  Drop It Off 

You may hand deliver your rgistration at the address above.  Please enter door #24 and if no one is available to take your registration, simply dorpit in our mail slot in our office door #1002.  After the office closes at 4:30 pm, you may still hadnd deliver your registration at the above address. 

The Senior High School door #24 will be open as follows:

       5:35-am- 8:45 pm  Monday - Thursday  

       5:35 am-7:45 pm  Friday

       7:00 am-2:45 pm  Saturday & Sunday
